I have my own established templates, but I am flexible and can work with any template provided by the client. In my summaries, I include essential information such as chief complaints for each visit, medication history, family history, abnormal findings from physical exams, medical/surgical history, assessment evaluations, treatment plans, and evaluations of diagnostic reports like MRIs, X-rays, CT scans, ECGs, echocardiograms, EMGs, and more. I also provide a comprehensive assessment of motor vehicle accidents (MVAs) when necessary.
